After years of observing common obstacles that people face when providing care for a loved one with memory impairment, Oakwood Creative Care’s nonprofit organization founded The Dementia Hub to create a central source for dementia support in Arizona.
At The Dementia Hub, our team will help you navigate available options that can support you in your unique situation. You’ll find Memory Cafes, support groups, engagement activities, resources, connections, and educational opportunities. Participants in our COPE Dementia Coaching Program receive individualized assessments and training from our OT and PA*, using evidence-based strategies to achieve better results in the home setting while promoting an improved quality of life for the care partner and the person living with dementia.
Today, The Dementia Hub helps families pave a path that begins with a loved one’s diagnosis and continues along their entire journey with Alzheimers or other form of dementia…preserving a meaningful life with their loved ones and celebrating each moment of joy along the way.

The COPE Program
These evidence-based Dementia Coaching Sessions are for people living at home with dementia, and their care partner, to identify key areas which may be contributing to feelings of distress, or hindering quality of life. After an in-depth assessment of your situation, you’ll learn targeted strategies and tools to improve upon care challenges you’re experiencing in the home.

Community Events
The Dementia Hub hosts several events each month for people living with dementia, their families, and professionals. Check our calendar for upcoming CE opportunities, engagement and community for people living with dementia and their families, exercise and physical wellness classes, dementia experience simulations, and dementia friendly trainings.

Memory Café
The Mesa Memory Café meets Wednesdays from 9:30am – 11:00am, providing engagement and support for care partners and their loved ones with dementia. Enjoy a coffee and pastry while socializing with your loved one and others in the community, before breaking off into a support group for care partners and free engagement class for people living with dementia.
