GUIDE Lights the Way Through Dementia
The Centers for Medicare & Medicaid Services (CMS) created the GUIDE model to enhance the quality of life for people living with dementia, reduce caregiver strain, and enable individuals to remain living in their homes.
As a founding provider of GUIDE in Arizona, Oakwood Creative Care is proud to offer the complete menu of services covered by the program. These services include thorough assessments, care plan development, respite care through adult day clubs, and ongoing care navigation. Families have 24/7 access to a support line to connect with an OCC dementia specialist for advice or support with various dementia-related questions or challenges they encounter. Additionally, GUIDE participants have unlimited access to community support groups, training and educational presentations, and social events.
With the GUIDE model at Oakwood Creative Care, families can navigate dementia with confidence and focus on creating meaningful moments together at home.

*Adult Day Clubs are commonly-known as a Adult Day Care, though we call it a “club” to emphasize that its a fun and friendly environment with cognitive engagement activities. There may be alternate sources of funds available for Adult Day Clubs if your loved one is not elegible for GUIDE membership. Adult Day Club is also available seperatly through private pay.
**A doctor is on staff for urgent, non-ER needs, like UTI testing or antibiotic prescriptions. No scheduling appointments days or weeks later when your primary care or other doctor happens to have an opening. No waiting an hour or more to meet with the doctor once you arrive. No flustered, busy doctors addressing only some needs while ignoring what your loved one might need right now. This does not replace your primary care provider, it supplements your primary care and gives you timely response when you need it as long as it’s not a case for the Emergency Room.
***These workshops are still available through private pay for non-GUIDE and non-HUB members. See also the COPE program, which stands for Care of Older People in their Environment. The same Occupational Therapists who are part of the COPE program conduct these Problem Solving Workships. Occupational Therapists are trained, licensed professionals who help individuals with Activities of Daily Living (ADLs), ensure that the home environment is functionally safe to avoid trips and falls and other important aspects for the well-being of the individual with Dementia and family caregivers.
Eligibility Requirements for GUIDE Membership
✔️ Confirmed dementia diagnosis
✔️ Medicare as the primary payer
✔️ Enrollment in Medicare Parts A & B
✔️ Not currently utilizing Medicare hospice benefits
✔️ Not residing in long-term nursing home (assisted living facilities are not eligible)
