What is GUIDE?
The Guide program was spearheaded by the Centers for Medicare & Medicaid Services (CMS) to improve the quality of life for people living with dementia, reduce strain on caregivers, and help people remain in their communities. Guide offers care coordination, caregiver education and support, and respite services.
Despite its prevalence, many people living with dementia do not consistently receive high-quality, coordinated care. As a result, they often experience poor outcomes. Many caregivers for people living with dementia report high levels of stress and depression, which negatively affect their own health.
After a diagnosis, Guide delivers key supportive services to people with dementia, including comprehensive, person-centered assessments and care plans, care coordination, and 24/7 access to a support line. A care navigator will help you access services and support. The GUIDE Model enhances access to the support and resources that caregivers and people living with dementia need.
